Grant Application Requirements

All applications must be mailed to our address at the US Bank in Joplin, Missouri. In order to be considered for a grant, which we award at our annual meeting each year in October, applications must be received between January 1 and August 31. Due to the unpredictability of the mail and the time needed to process requests we ask all applicants to respect these time constraints in order to assist our volunteers in preparing the many requests we receive each year to present for consideration at the board meeting.

The notification of grant awards will be made as soon as possible after the meeting and we try notify all applicants within 30 days as to the disposition of their grant request. Again your patience is greatly appreciated.

 

Proposals for grants must include the following:

1. Mission statement and background of the organization.
2. Demonstration of need (What community is being served? What needs are being met by this project?)
3. Amount requested.
4. Project title and name of project director (or contact).
5. Purpose of program for which funds are sought.
6. Timelines for program implementation.
7. Method of required evaluation after completion of program.

 

The following attachments must also accompany requests:

1. Summary checklist
2. Current financial statement
3. Copy of federal tax exempt status
4. Roster of Board of Directors
5. Any supporting materials

 

Reports

A status report and evaluation of any funded project must be received by the Foundation either upon completion of the project or not later than May 31st of the following year. This is requested of all grant recipients without regard to the size of the grant.

The organization receiving the grant must acknowledge the amount received, the status of the project funded and how the funds were utilized.
